Atty. Libayan, I'm Mrs.Dionisio, we have a problem with my husband regarding his surname,Dionisio.His biological parents had him adopted to a certain chinese couple who was their neighbor.Tiongson was the surname.We were married as Tiongson coz we did't know yet that he was not legally adopted.Later,my husband discovere that he was a Dionisio when he checked his NSO record.My husband was informed by his biological father that he was registered upon birth as a Dionisio,so he acquired his authenticated birth cert. The problem is when we were ready to get our marriage cert.there was no record in the office where the priest registered us,but they were telling us that they were sure that it has been entered in.Regarding my daughters birth cert.she was born in Bulacan in the year 1986,then we transferred to San Pedro and graduated in the year,2003.So we were requested by the school to get her birth cert.but the midwife did not register her.So the NSO of San Pedro suggested that we get two disenterested parties to witness that she grew and was born in San Pedro instead.So she was registered and acquired her delayed birth cert. at the NSO.After a certain time,my daughter was already employed,and she was getting her copy of authenticated birth cert. there appeared her birth cert. from Bulacan which was submitted just then, so there were 2 birth cert. already. Atty. please advice us what to do. Thank you very much for your kind attention and God bless you and your staff always.

Just use the first birth certificate. The entries are the same anyway, only the issuing civil registrar.

It also helps to execute an affidavit pertaining to the matter so if ever there is a conflict, it can be easily resolved.
