The issue is that she was born illegitimate on September 3, 1988.
On her birth certificate she has the following name: Ailyn Ladioa Borac
Ladioa is her grandmother's surname, and Borac is her father's surname.
Her mothers name at the time of her birth was: Mariyln Ladioa Palce

Her parents married when she was 6 years old.
At that time her Mother's name changed to Marilyn Palce Borac.
At that time she took on the name Ailyn Palce Borac
Her supporting documents with the name Ailyn Palce Borac are: high school ID, high school dilpoma, form 137, driver's license, and NBA clearance
She was born in borongan samar eastern.

I read the law and understand that if she was born after August 3, 1988 she should have taken her mother's surname. Is this correct?

She is telling me that her birth certificate needs to match her supporting documents. She is saying that she needs to get a copy of her parent's baptismal certificates as they do not have birth certificates.

Please let us know what path needs to be taken and how to correct the issue. If we can agree on an amount I would like to retain you to take care of the issue.

Just file an Affidavit of Legimitation po with the local civil registrar and have the mistakes corrected.

This was very helpful. Thank You. I am looking at the supporting documentation now and was wondering if all three of these documents are required to successfully complete the certificate of validation?

If so are the names on the CTC required to be the same as on the birth certificate?
Lastly, how do I get the certificate of No Marriage of both parents.



a. Certified True Copy (CTC) of the Certificate of Live Birth of the child;
b. CTC of the Certificate of Marriage of Parents;
c. Certificate of No Marriage of both parents;
